Membership Forms

Membership Application

Download this packet if you are interested in purchasing one of our available memberships. In the packet you will find the following forms:

  • Membership application: In addition to your information, please be sure to select the type of membership you wish to purchase. If you are purchasing a family membership, please include the requested family member information on the form.
  • Health History (optional): On this form are questions about your health. Although optional, we ask that you complete this form so that we may be able to better assist you and make your experience here as welcoming as possible.

Liability Waiver

If you are coming to the SpoFit for an event, you will need a signed liability waiver on file before participating. Save some time by downloading it here and signing it before coming in.

Scholarship Application

You may be eligible for reduced membership costs. If you are on a fixed income and our membership prices are outside of your budget, please feel free to download the application and submit it along with your membership application. You will be required to provide documentation of income for your scholarship application to be considered.
